  I tried before but my understanding is to make a less buerocratic (sp ) system .Commisioned by the people who pick up the peices most often ie the GPs , who can respond to local needs and wishes , identified by close active liasion with groups that represent the community they serve ie councils . This should ensure that there is less of a gap between health and social care and mean that it is more joined up , responsve and accountable . But as I said before as with ANY change the real impact will not be certain until it is up and functioning
