It seems to me to be part of that over arching governmental need to control everything inappropriately to the detriment of judgement and creativity. There must be safeguards - I wouldn't want to go back to a time when routine abuse, for example, of mental health patients was tolerated. But if we consider the Castelbecks and Southern Crosses, the hog-tying of health and social services in red tape and "targets" and the emphasis on box ticking over care has not reduced risks by that much. True we are more aware now, but there is far more hidden abuse out there than we would comfortably expect. The system, as you rightly say, has increased "checks" without driving up standards exponentially.
Unfortunately the tenet of "if it is not written down it did not happen" has led to things being written down but not done!
You cannot really do this writing whilst you are going round working since it means accessing each client's indiviual file.
I have had some experiences this last year which were not absolutely positive.
I hear the understatement in that Diana............

Some very realistic postings to the dilemma of delivering frontline care.
Nursing has gone in many differing directions but the basis of it is surely care.
