A well exposed flash photo of the deep pile rug in someone's dining room (why would they take that?).
An underexposed flask photo of a dog wearing a reflective collar.
Put these together with the wonders of Photoshop to produce a shaggy dog story.

I missed that one sorry. I'd say a double exposure if it was film (as Ray said) or a deliberate fake. Yes Howard it is relatively easy to do.
