    Where its appropriate it really does work , Patients have reported they feel more involved and there isnt the long delay in getting the right treatment . Its less traumatic than being moved in and out of hospital and also cuts the risk of cross infection, Couple this with the Chronic condition support that the Community Matrons can access and the intermediate care teams it might just be a way forward
