It would be very difficult for a doctor with ethics to refuse to treat anyone, surely they could hold the passport as a kind of surety.
The ideal solution would be to only allow foreigners into the country with adequate insurance cover.

Unfortunately it is easy to have more than one passport for some people, some years during the asylum seekers time here one got pulled up by the police in Malvern Rd for erratic driving. When asked to see his licence the inebriated driver pulled out 3 passports from 3 different countries all with his photo in.
