    My personal view for what it is worth, is that the NHS as currently constituted is an unmitigated disaster.

    We need to
    A) recognise that the worst people to dictate how healthcare in this country is run and organised in this country are the Department of Health
    B) recognise that the second worst set of people to run it are politicians
    C) realise that consultants are not gods, nor are they business people
    D) redefine the role of the Department of Health as defining policy and minimum standards of care BUT NOT measuring those
    E) break up the monolithic NHS into much smaller local healthcare providers with a small number of regional or national centres of excellence that provide teaching services, accreditation and speciality care
    F) gear services to the needs of the local population and allow them to be controlled by a mix of local people and local healthcare professionals
    G) acknowledge that the actual entity that delivers this is less important than the actual delivery - so it could be a local trust, a community interest body, a charity or even a private company
    H) Finally accept that the days of the open cheque are long gone and like everything else healthcare needs to operate within the budget it is set
