Actually, I have no problem with the Matron approach, and I do think that tendering out services in Thatchers day was the star of the rot, in combination with an intransigent union group. I was a hands on nurse back in those days and watched the cleaning services plummet following the changes. When the cleaners were part of the ward team and under the Matron (so to speak! oo-er!) they performed well and took a pride in their ward - it was "their" territory and Matrons smile worked wonders - as did her frown. We were clear that Thatcher, plus the unions determination to be difficult rather than strategic, spelled the end of the NHS even back then. We were proved right - standards have dropped like a stone since then despite all the safeguards and standards and procedures and groups set up to monitor things. All to no avail.
