Like Jenni I always say a happy 'Hello' to grumpy looking cashiers (or anyone like that in a public place.) It annoys them, but if they are simply having a bad day then maybe I'll have cheered them up a little. Also a little compliment always goes a long way. I do find most cashiers are very pleasant and we have a chat. I often find it's the other shoppers that are grumpy not the cashiers!
Having cashiers/shop employees continue chatting to their friends whilst I am trying to pay or ask a question only results in one outcome. If I am ignored for longer than a reasonable about of time (i.e. I felt they genuinely didn't see me or it was a matter of life or death) I simply walk out - often leaving the item on the cash desk.
